Managing and Learning from Incidents

 Introduced in 2016 in response to changes in the reporting of Incidents for the purposes of revalidation for doctors in training the process outlined below was introduced.

The primary driver was to ensure all doctors have access to supervision and support when receiving Clinical Incidents whilst also identifying individuals learning and opportunities for wider education.
